Replace an application's outbound SCIM provisioning

A whole-object replace: fields left out take their defaults, except the bearer token, which is kept (required on the first PUT and when baseUrl changes). Changing baseUrl starts over at the new URL; nothing is deprovisioned at the old one. Take If-Match with the settings' ETag.
